Speaker	:	Dr CHIANG Chung Seung, Sammy Dr Chiang is at present Consultant Cardiologist in Private Practice, Honorary Clinical Associate Professor of University of Hong Kong. He was Past President of the Hong Kong College of Cardiology, Head of Cardiology of Queen Elizabeth Hospital and Chairman of the Central Cardiac Committee of the Hospital Authority. He was awarded Fellow of the Royal College of Physicians of London, Edinburgh and Glasgow and Fellow of the American College of Cardiology in 2000 – 2003. He was the Chairman of the Organizing Committee of the 8-26th Annual Scientific Congress of the Hong Kong College of Cardiology since 2000. Dr Chiang’s main interest in Cardiology are Percutaneous Coronary Intervention and Electrophysiology Study and RF Catheter Ablation.

Details	:	Introduction: As a single disease entity, ischemic heart disease is the number 1 killer in Hong Kong. CT coronary angiogram has become the most popular test for diagnosis of ischemic heart disease. Coronary risk factor modification, antiplatelet therapy and percutaneous coronary intervention are commonly used in the treatment of ischemic heart disease. About “60 Fan Club” 60 Fan Club (樂齡俱樂部) is a branding of the HKIS Senior Members Committee (SMC), for promoting SMC events including but not limit to seminar, visit, workshops. Remarks	:	Special arrangements under COVID-19 Pandemic: Prior online registration is required. Walk-in registration will not be entertained. Limited number of seats with social distancing is available at the Surveyors Learning Centre. Participants should scan the “LeaveHomeSafe安心出行” QR code with their mobile phones or register their personal information in the online Health Declaration Form (https://forms.office.com/r/Q7vDYmtfxQ) upon arrival at the Surveyors Learning Centre. Participants will be denied entry if they fail to scan the “LeaveHomeSafe安心出行” QR code or complete the online Health Declaration Form. Participants will be denied entry to the Surveyors Learning Centre if they have been travelling abroad, or have been in direct contact with someone who has tested positive for COVID-19, or have any symptoms of COVID-19 (including but not limited to fever, cough, shortness of breath, breathing difficulty, sudden loss of taste or smell and/or sore throat) in the last 21 days, or have been confirmed with COVID-19 or undergoing compulsory quarantine or medical surveillance arrangements of the HKSAR Department of Health. Participants are required to wear a surgical mask at all times at the Surveyors Learning Centre. Eating and drinking is not allowed at the Surveyors Learning Centre. Disinfection service has been arranged at the Surveyors Learning Centre (comprising the Lecture Room, Meeting Room 1, Board Room, Library and Meeting Room 2).

1.	The registration fee is non-refundable and non-transferrable.
2.	A registration number will be generated for each application. The registration number is unique and non-transferrable. Members cannot proceed to payment if they do not apply the CPD event and obtain the registration number through the HKIS website.
3.	Payment can be made by cheque or by Credit Card (Shanghai Commercial Bank Ltd. / American Express). Cheques should be made payable to “SURVEYORS SERVICES LTD.”. A separate cheque or Credit Card payment instruction form is required for each event/ application. You should write down the registration number and HKIS membership number at the back of your cheque. The HKIS Secretariat will update the payment status and your application will be confirm when HKIS Secretariat receive your payment.
4.	Payment by PayPal is also acceptable. Please register in our web site before the closing date for each event. Please be reminded that if you cannot settle the payment within 7 days from the date the HKIS Secretariat accepted your registration, your registration number will be invalid and your application will be rejected. Your name will not be included in the registration list and you cannot attend the concerned CPD event. (Not applicable for site visit / social event / joint event with other organisation)
5.	An official receipt which must be presented at the event, will be available for downloading from the "CPD Profile" under Members Corners when payment is received.
6.	Incomplete or wrongly completed registration will not be processed.
7.	In the event a Typhoon Signal No. 8 or above or Black Rainstorm Warning is hoisted, the event will be postponed and a new arrangement will be announced. Should the aforesaid warnings be lowered 4 hours before the event, the event will proceed as normal.
8.	If you have not received any reply from HKIS Secretariat within 7 days of the event, you may call the Secretariat at 2526 3679 to check the progress of your registration.
9.	The HKIS reserves the right of final decision and interpretation in the case of any disputes.

Under no circumstances shall The Hong Kong Institute of Surveyors ("HKIS"), their staff or agents be held liable for the consequence of any accidents whether or not they are caused by the negligence of the HKIS, their staff and/or agents.
2.	Members participating in the above visit shall be in good health condition.
3.	While the HKIS has arranged Group Personal Accident insurance for members joining the above visit, members who have been accepted for joining the above visit can consider procuring an appropriate insurance policy to cover their own risks before attending the event.
4.	Claims related to COVID-19 and/or pandemics are excluded under the insurance policy procured by the HKIS.
