Speaker	:	Dr. CHUI Wing Ho William Dr. CHUI Wing Ho William is a psychiatrist in private practice. He graduated from Faculty of Medicine, University of Hong Kong. He obtained Membership of the Royal College of Psychiatrists in the United Kingdom and Fellowship of Hong Kong College of Psychiatry. Dr. CHUI is also an Honorary Clinical Assistant Professor at the Department of Psychiatry, the University of Hong Kong, and an adjunct Assistant Professor at the Centre of Buddhist Studies, the University of Hong Kong. 崔永豪醫生是一位精神科專科醫生，現私人執業。他畢業於香港大學醫學院，其後獲得英國皇家精神科醫學院院士及香港精神科醫學院院士。崔醫生亦是香港大學精神醫學系榮譽臨床助理教授，以及香港大學佛學研究中心客座助理教授。

Remarks	:	**This CPD talk is free of charge for HKIS QSD members. If registered QSD members fail to attend the CPD talk on 20 July 2022, the HKIS will send an invoice of HK$150 to those no-show members for settlement. And the no-show members may not be allowed to register the coming free QSD CPD talk. The event will be conducted online via ZOOM. Upon successful registration, the ZOOM link for joining the CPD event will be available in the CPD/PQSL profile under the "Member Area" of the HKIS website. Please install the Zoom Application in advance at https://www.zoom.us/download. For Mobile phones, iPads & Android, install the ZOOM Cloud Meetings app from App Store or Play Store. For PC and Mac, download and install the Zoom Client for Meetings.

1.	The registration fee is non-refundable and non-transferrable.
2.	A registration number will be generated for each application. The registration number is unique and non-transferrable. Members cannot proceed to payment if they do not apply the CPD event and obtain the registration number through the HKIS website.
3.	Payment can be made by cheque or by Credit Card (Shanghai Commercial Bank Ltd. / American Express). Cheques should be made payable to “SURVEYORS SERVICES LTD.”. A separate cheque or Credit Card payment instruction form is required for each event/ application. You should write down the registration number and HKIS membership number at the back of your cheque. The HKIS Secretariat will update the payment status and your application will be confirm when HKIS Secretariat receive your payment.
4.	Payment by PayPal is also acceptable. Please register in our web site before the closing date for each event. Please be reminded that if you cannot settle the payment within 7 days from the date the HKIS Secretariat accepted your registration, your registration number will be invalid and your application will be rejected. Your name will not be included in the registration list and you cannot attend the concerned CPD event. (Not applicable for site visit / social event / joint event with other organisation)
5.	An official receipt which must be presented at the event, will be available for downloading from the "CPD Profile" under Members Corners when payment is received.
6.	Incomplete or wrongly completed registration will not be processed.
7.	In the event a Typhoon Signal No. 8 or above or Black Rainstorm Warning is hoisted, the event will be postponed and a new arrangement will be announced. Should the aforesaid warnings be lowered 4 hours before the event, the event will proceed as normal.
8.	If you have not received any reply from HKIS Secretariat within 7 days of the event, you may call the Secretariat at 2526 3679 to check the progress of your registration.
9.	The HKIS reserves the right of final decision and interpretation in the case of any disputes.

1.	I confirm that I fully understand and accept the risk of joining the above visit. Under no circumstances shall The Hong Kong Institute of Surveyors ("HKIS"), their staff or agents be held liable for the consequence of any accidents whether or not they are caused by the negligence of the HKIS, their staff and/or agents.
2.	Members participating in the above visit shall be in good health condition.
3.	While the HKIS has arranged Group Personal Accident insurance for members joining the above visit, members who have been accepted for joining the above visit can consider procuring an appropriate insurance policy to cover their own risks before attending the event.
4.	Claims related to COVID-19 and/or pandemics are excluded under the insurance policy procured by the HKIS.
